Kophos

Kophoses are pain purists who believe that screams and tears detract from the experience of suffering. These velstracs gouge out their eyes and cut out their tongues to make such outbursts impossible, covering the empty cavities with patches of their own skin or spiked lumps of metal, though they are still disturbingly able to see and speak. Beyond these self-inflicted injuries, they resemble typical humans, though their bodies are covered in countless gashes and scars that slowly, gruesomely heal themselves. They wrap themselves in dozens of barbed chains.

Kophoses consider it their purpose to find mortals who either inflict physical suffering on others or who are plagued with constant pain due to medical conditions and teach them how to be silent and stoic in their agony.

Silent Agony (aura, divine, emotion, fear, mental, visual) 30 feet. When a creature ends its turn in the aura, it feels as if its mouth is sealing shut with excruciating pain. The creature must succeed at a DC 19 Will save or become stupefied 2 for 1 round. On a critical failure, it also can't make any sound with its mouth for as long as it's stupefied. This prevents it from using sonic or auditory actions that require its mouth, casting spells, and using linguistic actions. Velstracs are immune to this effect.

Bloody Epiphany A creature hit by a kophos's chain scourge Strike must succeed at a DC 21 Fortitude save or take 2d6 persistent bleed damage. For as long as it is taking this persistent damage, a creature is engrossed by the pain from its own wounds and is slowed 1. A creature stupefied by the kophos's silent agony can't willingly attempt to stop this bleeding, on itself or others. A creature that critically succeeds at its Fortitude save is temporarily immune to bloody epiphany for 24 hours.Feed on Pain [two-actions] (concentrate, divine, mental) Requirements Three or more creatures within the kophos's silent agony aura are stupefied, slowed, or frightened; Effect Each of the required creatures takes 5d6 mental damage (DC 21 basic Will save), and the kophos regains a number of Hit Points equal to the highest amount of mental damage taken.Focus Gaze [one-action] (concentrate, divine, emotion, fear, mental, visual) The kophos stares at a creature it can see within 30 feet. Sidebar - Additional Lore The Purest Pain

The touch of a kophos is one of the few things that can let other velstracs experience novel sensations of pain. As such, a kophos might rise as a leader of velstracs who are almost religiously devoted to them. Such cults can be found in remote corners of the Netherworld and may sometimes try to impose their mockery of faith on the mortal Universe.

The shadow-dwelling fiends known as velstracs all share a horrifying preoccupation with the search for ultimate sensation through self-mutilation. Velstracs transcend their stoic detachment only when inflicting pain and terror upon their victims, practicing new forms of torture, or turning their agonizing practices back on themselves. They consider themselves enlightened beings, transcending such limitations as morality or mortal taboos, but their victims know them as emotionless tormentors who inflict sadistic suffering. These fiends claim to seek perfection in thought, form, and action, although they don’t recognize any refinement that doesn’t require the painful excision of the flesh or spirit.

Velstracs manifest from the souls of the most extreme masochistic or sadistic mortals who are judged and sent on to the Netherworld. They take on forms that suit their vile predilections, ranging from the low-ranking augurs to the maestros of suffering and mutilation, the eremites. The process of transformation warps the soul step by step, with other velstracs conveying their new members through untold chambers of pain among the dark reaches of the Netherworld.

Velstracs are creatures of pain and mutilation. Encounters with velstracs often delve into the realm of body horror, which could be overly disturbing to some players, leading to unintentional effects on your game and your friends. Before introducing velstracs into your game, talk with your players to learn about any particular phobias or limits to determine what content you bring to the table, and consider that on your own behalf as well. Some specific elements to be aware of are velstrac themes around cutting flesh and drawing blood, especially the eremite’s abilities to remove the flesh of others and graft it to themself.

Velstracs originated with the first debased thoughts of mortals, which divine beings found so deplorable that they locked all velstracs away in Hell. It wasn’t long, however, before the newly formed beings escaped their infernal prison to the Netherworld.

Many velstracs outsource the forging of their chains to unique velstracs known as almoners. These six-armed, legless humanoids toil endlessly at a great burning foundry called the Shadow Forge. Almoners operate under mysterious strictures and are just as likely to turn a velstrac away emptyhanded as not.
